What is compounding?
Compounding is the process of a licensed pharmacist or physician preparing a customized medication for an individual patient. A compounded medication differs from a commercially manufactured drug in that it is made specifically for one patient's needs — a different strength, dosage form, or ingredient combination than what's available commercially.
Common reasons for compounding include: allergies to inactive ingredients in commercial products, medications that need to be in a liquid or topical form, dosage strengths not commercially available, and discontinued medications that are still clinically needed.

503A vs. 503B: What's the difference?
503A pharmacies are traditional compounding pharmacies that require a patient-specific prescription. They are regulated by state boards of pharmacy and must comply with USP <795> (non-sterile) and USP <797> (sterile) standards. They are exempt from full FDA cGMP requirements.
503B outsourcing facilities are FDA-registered, cGMP-compliant facilities that can compound in bulk without individual patient prescriptions. They supply hospitals, clinics, and surgical centers for "office use." They undergo regular FDA inspections and biannual product reporting. 503B facilities are the institutional gold standard for compounded drug supply.

What is PCAB accreditation?
The Pharmacy Compounding Accreditation Board (PCAB) is the independent accreditation body for compounding pharmacies. PCAB accreditation is voluntary but signals that a pharmacy has undergone a rigorous third-party quality audit — including staff qualifications, facility standards, documentation practices, and quality assurance systems.

Peptide compounding: What to look for
Peptide compounding has grown significantly in longevity medicine and performance optimization. Key quality indicators when sourcing a compounding pharmacy for peptides include: 503B registration (preferred for injectable peptides), Certificate of Analysis (CoA) from an independent laboratory, HPLC purity data (>98% purity standard), endotoxin testing (<0.5 EU/mL for injectables), and E-Beam or Gamma irradiation sterility assurance per ISO 11137.
Always ask the pharmacy for the source of their API (Active Pharmaceutical Ingredient) and verify that it is sourced from a cGMP-certified supplier with a full CoA.
